Application Please use the following form to apply. If you encounter any problems with the application form, click here to send us an e-mail message with your request for DG certification. Provide your name, the correct URL of your website, your e-mail address, and a statement that you are at least 18 years of age. (Click on the "legal notices" button to the left if you would like to review our privacy policy before submitting your application.) We will review your website within four weeks after we receive your application. If your site wins, we will notify you within that time period.
